MSWA in Albany is seeking compassionate and reliable Support Workers to join our Albany team. This role combines residential and community-based support, providing person-centred care to clients with high and complex needs.

You''ll work across both settings, supporting clients in their homes, within the community, and in residential environments to help them achieve greater independence and quality of life.

Formación

• Certificate III in Individual Support, Disability, or Aged Care is required.
• NDIS Worker Screening Check is required.
• First Aid and CPR certificates current.
• Australian Driver's Licence required.
• NDIS Screening Clearance (or willing to obtain).
• Able to work a rotating roster including mornings, afternoons, nights, weekends and public holidays.

Responsabilidades

• Provide personal care and support with activities of daily living (ADLs).
• Support continence management and medication administration.
• Assist with transfers, mobility and hoist use per care plans.
• Deliver care to clients with complex needs.
• Help clients participate in community activities and appointments.
• Provide transport assistance when needed.
• Monitor wellbeing and report changes to supervisors.
• Maintain accurate documentation and follow care plans.
• Collaborate with team to ensure quality care.

About the Role

We're looking for compassionate and reliable Support Workers to join our Albany team. This unique role combines residential and community-based support, providing person-centred care to clients with high and complex needs. • Support clients with continence management and medication administration.
• Assist with transfers, mobility support and hoist use in accordance with care plans.
• Deliver high-quality care to clients with complex care needs.
• Support clients to participate in community activities, appointments and social engagements.
• Provide transport assistance where required.
• Monitor and report changes in client wellbeing and elevate concerns appropriately.
