  • Maintain electronic filing systems and records.
  • Deliver excellent customer service to patients and stakeholders.
  • Work collaboratively with clinicians, managers and administration teams.

The Planned Care team work across the Southern district and encompasses all aspects of non-acute health care delivered across primary, rural and hospital settings.
