Job summary

APM New Zealand in Hamilton is seeking a passionate Physiotherapist to join our Community Rehabilitation Services across client homes, workplaces, and communities. You will provide rehabilitation and support within a multi-disciplinary team, focusing on vocational and community rehab goals.

Qualifications

• 2+ years of experience in vocational rehabilitation and community rehabilitation services

About The Role

Reporting to your Team Leader, you’ll be part of a vibrant team culture that supports and empowers each other every day.

Community Rehabilitation Services

• Provide rehabilitation and support to individuals within their community
• Work within a multi-disciplinary team to help clients achieve their rehab goals
• Knowledge of community services pathways
• Experience writing high-quality reports and justifying clinical reasoning

Physiotherapy Services

• Experience in musculoskeletal, neurological and vestibular rehabilitation
• Experience in delivering gym-based, home-based, and functional rehabilitation programs
• Skilled in assessing movement patterns and progressing strength and mobility

Aged Care

• Mobility and Transfer Assessments: Carrying out assessments for new residents in both Hospital-Level Care and Rest Home units.
• Collaboration with the Physiotherapy Assistant: Work closely with the Physiotherapy Assistant, offering guidance on how they can provide ongoing support and regular input to residents in line with the mobility and maintenance goals.
• Ongoing Support for Hospital-Level Residents: Closely monitor the mobility needs of each resident admitted under Hospital-Level Care, to help maintain their functional abilities, and recommend updates to care plans when needed.
• Manual Handling Training: Delivery of training sessions to care and auxiliary staff, as requested by the facility, to ensure safe and effective manual handling practices.
• Create, plan, monitor, and progress rehabilitation programmes tailored to the needs and goals of older adults in an aged care setting
