Job summary

Advanced Personnel Management in Hamilton is seeking a Physiotherapist to deliver community-based rehabilitation and multidisciplinary care. You will assess, plan, and implement rehabilitation programmes for diverse client groups across homes, clinics, and community settings.

About the role

Reporting to your Team Leader, you’ll be part of a vibrant team culture that supports and empowers each other every day.

Community Rehabilitation Services

• Provide rehabilitation and support to individuals within their community
• Work within a multi-disciplinary team to help clients achieve their rehab goals
• Knowledge of community services pathways
• Experience writing high-quality reports and justifying clinical reasoning

Physiotherapy Services

• Experience in musculoskeletal, neurological and vestibular rehabilitation
• Experience in delivering gym-based, home-based, and functional rehabilitation programs
• Skilled in assessing movement patterns and progressing strength and mobility

Aged Care

• Mobility and Transfer Assessments: Carrying out assessments for new residents in both Hospital-Level Care and Rest Home units.
• Collaboration with the Physiotherapy Assistant: Work closely with the Physiotherapy Assistant, offering guidance on how they can provide ongoing support and regular input to residents in line with the mobility and maintenance goals.
• Ongoing Support for Hospital-Level Residents: Closely monitor the mobility needs of each resident admitted under Hospital-Level Care,to help maintain their functional abilities, and recommend updates to care plans when needed.
• Manual Handling Training: Delivery of training sessions to care and auxiliary staff, as requested by the facility, to ensure safe and effective manual handling practices.
• Create, plan, monitor, and progress rehabilitation programmes tailored to the needs and goals of older adults in an aged care setting
