We are looking for an organised, relationship-focused and proactivePHO Operations Co-ordinator to join our Community Care PHO team.
This is a key role supporting the day-to-day relationship between the PHO and our general practice network. You will work closely with practice owners, managers, GPs, nurses and wider practice teams to ensure PHO programmes, services and operational processes are well communicated, effectively implemented and supported across the network.

About the role
Reporting to theHead of Community Care PHO, you will be a key point of contact for practices across the network, providing practical advice, operational support and coordination across PHO services and programmes.
You will build strong relationships with practices, identify opportunities for improvement, support quality initiatives and help ensure practices have the information and support they need to successfully deliver PHO programmes.
The role combinesrelationship management, programme coordination, communication, quality improvement, administration and event coordination, making it ideal for someone who is organised, solutions-focused and confident working across a diverse stakeholder group.

About you
You will be a confident and highly organised professional who enjoys building relationships, coordinating activity and supporting people to achieve shared outcomes.
You will ideally have:
Experience withgeneral practice operations and management, as well as knowledge of primary care and population health frameworks, would be highly desirable.
This is a Permanent Full Time position working 40 hours per week across 5 days - Based in our head office in Parnell.
