Job summary

PHO Network Lead – Community Care & Arataki PHO is sought to design, implement and continuously improve primary health care services across the PHO network, improving access, equity and health outcomes for communities.

Reporting to the Head of PHO, you will coordinate with practices, Health NZ, iwi and Māori health providers, community organisations and internal teams to turn priorities into sustainable models of care with measurable impact.

Qualifications

• A relevant tertiary qualification in health management, public health, nursing, health sciences, business management or project management.
• Experience working in primary health care, PHO, NGO, community health or public health environments.
• Proven experience leading change initiatives and managing projects from concept through to implementation and evaluation.
• Strong written communication skills, including the ability to develop business cases, proposals, reports and recommendations.
• Experience building and managing effective relationships with a diverse range of stakeholders.
• Confidence interpreting and using health and performance data to inform decision-making and reporting.
• Strong organisational skills and the ability to manage multiple priorities and deadlines.

Responsibilities

• Lead service development and innovation across the network.
• Identify gaps and develop innovative, evidence-based solutions.
• Support national and regional health priorities, frameworks and funding initiatives.
• Develop business cases, service proposals and implementation plans.
• Identify opportunities to improve delivery of primary care services across the network.
• Monitor progress against milestones, budgets and outcomes.
• Lead change management and communications with practices and stakeholders.
• Monitor funding utilisation and prepare leadership reports.
