Job summary

Health New Zealand in Napier is seeking a Registered Nurse or Allied Health professional to join the Needs Assessment and Service Coordination (NASC) team. You will assess and plan services for older people and those with long-term health conditions to help them live safely at home.

Based in Napier, the role blends office work with community engagement, building relationships with clients, whānau, and providers to improve equitable access to support services.

Qualifications

  • Current New Zealand professional registration and a current Annual Practising Certificate.
  • Engage in thorough assessments and develop care plans.
  • Commitment to Te Tiriti o Waitangi and improving outcomes for Māori.
  • Excellent time management and ability to work independently and collaboratively.
  • Passion for supporting older people and those with complex health needs.
  • Experience in needs assessment, service coordination, community health, or care planning.
  • Working knowledge of InterRAI.
  • Experience with older persons and/or people with complex health and social needs.

Responsibilities

  • Undertake comprehensive clinical assessments with clients in their homes or community settings.
  • Develop care plans and make recommendations for supports and services.
  • Collaborate with whānau, healthcare providers, and community organisations to coordinate care.

About the Role

We are seeking either a Registered Nurse or Allied Health professional to join our Needs Assessment and Service Coordination (NASC) team. In this rewarding community-based role, you will work alongside older people and those living with long-term health conditions to help identify their needs and outline what support services are available to support services that enable them to live safely and independently in their homes.

Working closely with clients, whānau, healthcare providers, and community organisations, you will undertake a comprehensive clinical assessment, develop care plans, make recommendations for support services. This role offers the opportunity to build meaningful relationships, advocate for equitable access to services, and make a genuine difference in the lives of people across Hawke's Bay.

Based in Napier, you'll enjoy a role that combines office-based work with community engagement, while living in one of New Zealand's most sought-after regions, renowned for its sunny climate, coastal lifestyle, and vibrant community.

About You
