About the Role

The role of Lakes Needs Assessment Service Coordination (NASC) is to complete holistic assessments of the needs of our clients, and coordinate support services for older adults with health and disability related needs, and some younger people whose needs relate to a chronic long term health condition. We also work with clients who have recently discharged from hospital, as well as palliative care clients. Our aim is to support our clients to continue to live safely at home, or enable them to move into Residential Care if needed. The Lakes NASC team works with clients across the Lakes rohe (Rotorua, Mangakino, Taupo, Turangi). The Lakes NASC Team Leader will be responsible for developing and implementing changes in the service to align with Health NZ Lakes direction for the NASC service, as part of our Lakes Integrated Community Health Service. This exciting role provides the opportunity for a person with the appropriate skills to lead the service and drive the necessary developmental changes while also ensuring business as usual. The Team Leader is accountable for the activities, services, resources and results of their team. The Team Leader provides leadership and direction to their team through financial and operational management, monitoring of team performance and role modelling a professional, collaborative and client centred focused approach to their team. The team leader will ensure the NASC is responsive to the changing health environment by leading a NASC team that has the capabilities of delivering collaborative and coordinated approach to NASC clients in the future.

About you
  • Have New Zealand Registered Social Worker, Occupational Therapist, Physiotherapist or Nurse.
  • Have current New Zealand APC in the appropriate scope.
  • Have a Health qualification with a current New Zealand practicing certificate and with the equivalent of at least five years recent relevant experience.
  • Be a competent clinical professional capable of managing a team of varied clinical professionals and working collaboratively with other multidisciplinary teams.
  • You will have a broad range of experience both clinical and managerial.
  • Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth is desirable as is experience in projects / initiatives which give effect to Te Tiriti principles and frameworks, and the application of Mātauranga Māori and Kaupapa Māori approaches, particularly as they apply in healthcare settings.
