Job summary

Lakes NASC in Taupo is seeking a Needs Assessor Service Coordinator to join our multidisciplinary team. The role focuses on holistic assessments and coordinating support for older adults with health and disability needs, with cross-over work to Rotorua.

You will assess client needs, coordinate services, and support transitions from hospital or to residential care, working with clients across the Lakes region. This is a 0.6 FTE, part-time position based in Taupo.

Qualifications

• Proven assessment and clinical reasoning skills.
• Experience working in partnership with clients and whanau to meet support needs.
• Strong communication and relationship-building abilities.

Responsibilities

• Complete holistic assessments of clients' needs.
• Coordinate support services for older adults with health and disability related needs.
• Work with clients recently discharged from hospital and those requiring palliative care.
• Collaborate across Lakes region to support safe home living or transition to Residential Care when needed.
