Healthcarenz Limited is seeking an Intensive Reintegration Facilitator to join our team. This permanent, full-time role is community-based and covers the New Plymouth and Palmerston North areas.

You will support whaiora with complex needs to develop independent living skills and achieve personal goals, including budgeting, cooking and household management. You will work from a strengths-based, trauma-informed approach and collaborate with whānau, colleagues, and external agencies to promote

Qualifications

• Experience supporting whaiora with complex needs and challenging behaviours.
• Experience in supported accommodation, residential services or community support.
• Experience collaborating with Corrections, Probation Officers, Mental Health Services, Social Workers and other external agencies.
• Experience managing interpersonal conflict and maintaining safe living environments.

Responsibilities

• Support whaiora to develop and maintain independent living skills.
• Assist with daily living activities: budgeting, cooking, appointments and household management.
• Develop individualised support plans based on whaiora needs, strengths and goals.
• Promote social inclusion, community participation and meaningful activities.
• Maintain accurate case notes, incident reports and support plans.
• Work with colleagues, whānau and external agencies to provide safe, person-centred support.
