Job summary

MINDSG LTD in New Zealand seeks a Transition Support Officer to provide hands-on support, training and guidance to clients moving through various transitions, from school to community hubs, and between care settings.

The role supports the Hub Manager and a multidisciplinary team, helping develop and review Individualized Development Plans (IDP) and coordinating family involvement to promote person-centred growth and independence.

Qualifications

• Minimum Diploma in Social Work, Psychology, Community Services or other relevant fields with at least 2 years working experience.

Responsibilities

• Develop, review and maintain IDP for clients using a person-centred approach.
• Coordinate Transition Planning meetings and IDP reviews with AHPs and stakeholders.
• Provide hands-on transition training and document progress.
• Collaborate with Social Workers, Care Coordinators and therapists to support caregivers.
• Track progress and adjust IDP goals to promote independence.
