Job summary

Health New Zealand | Te Whatu Ora in Hamilton, Waikato, seeks a Community Support Worker to join Te Whakawhitinga in a community‑based team. The role focuses on practical support, goal setting and accessing housing, health and social services.

You will work with tangata hapori / taangata whaiora, uphold Kotahitanga values, and collaborate with other agencies to reduce tenancy risks and homelessness.

Qualifications

• Experience in mental health, community health or primary care settings.
• Ability to follow direction from registered staff to support clients toward health and social outcomes.
• Engages with Māori and Pasifika communities respectfully and inclusively.

Responsibilities

• Provide practical support to taangata hapori / taangata whaiora to achieve good health and social outcomes.
• Help identify goals and connect clients with housing, health and community resources.
• Collaborate with tangata hapori / taangata whaiora and other agencies in a transparent, coordinated way.
