Job summary

LinkPeople is seeking a full-time Housing Case Worker in Hamilton to develop and manage housing solutions for tāngata whaiora and whānau experiencing homelessness.

The role sits in the Ahikaroa Housing Central team, with a focus on collaboration across services to support transition, recovery and wellness. It suits a registered health professional with NZ APC, who can work at pace and uphold Te Tiriti o Waitangi principles.

Qualifications

• Registered health professional with current APC and NZ registration.
• Strong communication and relationship-building skills.
• Ability to work at pace and under pressure.
• Cultural awareness and commitment to supporting Māori and Pasifika whānau.
• Excellent attention to detail and proficiency in record keeping.
• Knowledge of Te Tiriti o Waitangi principles in work with Māori.
• Full New Zealand driving license.

Responsibilities

• Review and coordinate referrals from mental health residential services into independent housing options.
• Identify and overcome barriers to accessing supported independent living.
• Identify ongoing support needs for people in residential services to help them sustain and progress their living situation in line with their goals.
• Collaborate and navigate the social housing sector with other community providers.
• Build and maintain relationships with other community providers including mental health and addiction services.
• Provide clinical oversight support for staff and Kiritaki.
• Support Kiritaki with sustaining their tenancies in the community.
