Job summary

Careway is hiring a Care Coordinator for our Integrated Care Musculoskeletal programme based in Mt Eden, Auckland. You will manage a caseload, coordinate services across ACC, employers and healthcare providers, and ensure timely progression of rehabilitation and return-to-work plans.

You will be the central point of contact, staying calm under pressure and proactive in following up with the right people to keep recovery plans on track, contributing to client outcomes and team performance.

Qualifications

• Strong organisation and prioritisation skills.
• Clear and confident communication by phone, email and written notes.
• Ability to build productive relationships with clients, employers, providers and ACC.
• Solid problem-solving and sound judgement in time-sensitive situations.

Responsibilities

• Coordinate services and communication across clients, ACC, employers and healthcare providers.
• Set expectations with clients and providers to achieve pathway outcomes.
• Support return-to-work or independence pathways by planning and progressing next steps.
• Act as the primary contact for assigned clients throughout recovery.
• Monitor timeframes, outcomes and barriers to identify issues early.
• Keep accurate notes and timely written documentation.
