Job summary

Careway is hiring a Care Coordinator for our Integrated Care Musculoskeletal programme based in Mt Eden, Auckland. You will manage a caseload, coordinate services across ACC, employers and healthcare providers, and ensure timely progression of rehabilitation and return-to-work plans.

You will be the central point of contact, staying calm under pressure and proactive in following up with the right people to keep recovery plans on track, contributing to client outcomes and team performance.

About the Role

We are looking for a Care Coordinator to join our team based in the heart of Mt Eden village. The role sits within an innovative ACC-funded Integrated Care Musculoskeletal (ICPMSK) programme that supports people with knee, shoulder, and spine injuries, helping them progress through rehabilitation and return to work safely and sustainably (if they are off work due to their injury).

Reporting to a Team Leader, you will manage a caseload of clients, coordinate the services and keep communication moving between clients, ACC, employers, treatment providers (including vocational providers if appropriate) and medical specialists.

This is a role for someone who enjoys being the steady central point of contact in a busy environment. You will need to be proactive, calm under pressure and comfortable following up with the right people at the right time so the clients remain supported and recovery plans stay on track.

Your work will directly contribute to client outcomes, team performance and the ongoing growth and innovations of Careway pathways.

Key Responsibilities
  • Coordinate services and communication across clients, ACC, employers and healthcare providers
  • Set expectations with the clients and providers to achieve pathway outcomes
  • Support return-to-work or independence pathway by planning, checking on progress and managing the next steps
  • Act as the key contact for assigned clients throughout their recovery pathway
  • Monitor timeframes, outcomes and potential barriers so issues are identified early
  • Maintain accurate notes, documentation and timely written communication
About You

You may be a physio or occupational therapist who is looking for a change or someone who has experience in healthcare, claim management, recovery and rehabilitation, insurance, customer service, administration, retail leadership, financial services or another people-focused environment. Direct case management and clinical experience is helpful, but not essential.

What matters most is that you are organised, curious, emotionally steady and confident communicating with a range of people. You will be someone who follows through, asks good questions and takes pride in helping clients and stakeholders get clarity.

  • Practical problem-solving skills and sound judgement in complex or time-sensitive situations
  • Ability to work independently and autonomously while knowing when to escalation or seek support
  • Competence using Microsoft 365 tools, including Teams, Outlook, Excel and Word
  • A full New Zealand Driver Licence
Desired Experience

Helpful, but not essential:

  • Experience in case management, healthcare coordination or a comparable client-facing role
  • Familiarity with patient management systems or digital workflow tools
  • Exposure to ACC, vocational rehabilitation or rehabilitation-related processes
