Job summary

HarmonyCares seeks a Value Based Care Physician Leader to drive HEDIS and quality-focused outcomes for a high‑risk Medicare patient population. You will partner with physician and regional leaders to guide a clinical pod delivering home‑based primary care with a physician‑driven, team‑based model.

You will supervise APPs and collaborating physicians, enhance care coordination, and lead clinical education to improve chronic condition management and patient safety across multiple states.

Qualifications

• Medical degree required with license to practice in the applicable state.
• Board certification required for hires after Jan 1, 2026.
• CPR certification must be current and active.
• Unrestricted medical license and DEA registration required.

Responsibilities

• Lead clinical pod team on quality, patient experience and outcomes.
• Perform on-site supervision of APPs and collaborating physicians.
• Develop and monitor care plans for high‑risk patients and chronic conditions.
• Participate in chart audits and build a high‑reliability clinical culture.
• Collaborate with Regional Medical Director to drive care improvements.
