Job summary

Atrio Home Care, a partnership of Holland Home and Resthaven, seeks an RN Case Manager for in-home visits in Holland and surrounding counties. You will assess clients, plan care with physicians, and support families to maximize independence.

Flexible full- or part-time schedules, day shifts, with on-call rotation. Attractive compensation with weekend/holiday incentives and comprehensive benefits.

Qualifications

• Licensed RN in Michigan; BSN preferred.
• Valid Michigan driver’s license, proof of auto insurance, and reliable transportation for home visits.
• 1–2 years of acute care or skilled nursing experience; prior certified home health experience preferred.
• Passion for delivering high quality care to older adults.

Responsibilities

RN Case Manager | Home Visits, Grand Rapids & Holland/Lakeshore

Join Our Team as an RN Case Manager for home visits in beautiful downtown Holland! Atrio Home Care, the partnership of Holland Home and Resthaven, is seeking a compassionate RN Case Manager to join our Grand Rapids Holland/Lakeshore team. This position will involve providing direct care to clients in Ottawa and Allegan Counties.

Responsibilities

• Perform initial and comprehensive assessments and deliver compassionate care to clients
• Collaborate with physicians and other care team members to develop a Plan of Care
• Promotes client and family/caregiver independence through counseling and education
• Coordinate client discharge planning
• Identify barriers to desired outcomes and evaluates outcomes of care
• Prepare clinical documentation, progress notes in a timely manner using iPads for the EMR system
• Participates in on-call rotation

Full or part-time opportunities available

Schedule/Shift

• Monday - Friday
• Day shift 8-5 pm, with flexibility in scheduling to help you maintain a healthy work/life balance
