Job summary

HonorHealth is seeking an RN for Home Health with focus on comprehensive patient assessment and care coordination. The RN will document medical history, develop care plans, and work with the care team to achieve optimal patient outcomes in a home setting.

The role requires strong clinical knowledge, excellent communication, and the ability to manage medications, IV therapy, and patient education. Per diem days available, with day shift responsibilities.

Qualifications

• Bachelor's Degree in Nursing preferred.
• Graduate from an NLN/CCNE-accredited institution required.
• ADN/ Diploma hires must complete BSN within 60 months or have 10 years RN experience.

Responsibilities

• Follow ANA Standards for Professional Nursing Practice and Code of Ethics.
• Develop and document patient care plans and discharge planning needs.
• Administer medications and IV therapy as prescribed and monitor patient response.
• Coordinate with physicians and healthcare team to ensure high-quality care.
• Educate patients and families on health conditions and post-treatment home care.

Education

Bachelor's Degree in Nursing

NLN/CCNE-accredited institution graduation

Associate's Degree or Diploma in Nursing (BSN required within 60 months or 10 years RN experience)
