San Bruno Skilled Nursing in San Bruno, CA is seeking a dedicated Physical Therapist (PT) to join our full-time in-house rehabilitation team. The role offers a pay range of $55.00–$65.00 per hour, depending on experience, and welcomes new graduates.

You will evaluate residents, develop individualized treatment plans, and collaborate with an interdisciplinary team to improve mobility, strength, balance, and independence, while mentoring PTAs and promoting resident safety and quality of life.

Formación

  • Current and active California Physical Therapist (PT) license required.
  • Strong clinical assessment, treatment, communication, and documentation skills.
  • Ability to work effectively as part of an interdisciplinary team.
  • Compassionate, professional, and resident-centered approach.
  • Passion for geriatric rehabilitation and improving functional outcomes.

Responsabilidades

  • Evaluate residents and develop individualized physical therapy treatment plans.
  • Provide skilled physical therapy to improve mobility, strength, balance, coordination, and functional independence.
  • Establish measurable treatment goals based on each resident’s needs and abilities.
  • Monitor resident progress and adjust treatment plans as appropriate.
  • Document evaluations, treatment sessions, progress, and discharge recommendations accurately and timely.
  • Collaborate with nursing, occupational therapy, speech therapy, physicians, and other members of the interdisciplinary team.
  • Educate residents, families, and caregivers regarding therapy programs, safety, mobility, and home exercise programs.
  • Supervise and provide clinical guidance to Physical Therapist Assistants (PTAs), as applicable.
  • Promote resident safety, dignity, independence, and quality of life.
