Giving Home Health Care is seeking an experienced RN or LPN to provide in-home, patient‑centered care under a physician‑driven plan of care. The role is a 1099 contract with flexible scheduling and the potential for up to 84 hours per week, including 12‑hour shifts.

The patient is wheelchair‑bound and requires assistance with all ADLs and transfers; mileage reimbursement is available for eligible travel. Sign‑on bonus and steady hours provide a supportive, independent work environment.

Formación

• Graduate of an accredited nursing program (ADN/BSN).
• Current RN or LPN license in applicable state.
• Current CPR certification.
• TB test and physical examination required.
• Ability to lift up to 50 lbs and travel to patient homes.

Responsabilidades

• Execute physician prescribed plans of care.
• Complete compliant documentation of the care provided in the system of record.
• Manage and administer prescribed medications, treatments, and therapies.
• Conduct patient assessments and coordination of care.
• Perform activities of daily living for patients.
• Monitor vitals and GI intake and output.
• Assess skin integrity and administer wound care.
• Monitor for changes in patient condition and progression.
• Communicate with family, physicians, case managers, and others involved.
• Provide education and training to patient, family, and home health aide.
• Work one-on-one in the patient’s home to provide care.
• Collaborate with and supervise CNAs and PCAs.
• Perform other personal care services as necessary.
