Role Specifics

You will provide comprehensive direct patient care, including initial and ongoing assessments, development and updating of individualized care plans, medication administration, and daily clinical tasks aligned with best practices. You will monitor resident status, identify changes, collaborate with physicians and the interdisciplinary team, manage wound and skin integrity concerns, assist with activities of daily living, and document meticulously in the charting system to ensure continuity and quality.

Mentor & Leadership

You will be a mentor and partner to CNAs and other staff, promoting safety, infection control, fall prevention, and delirium risk management. The position offers substantial professional growth within geriatric and LTC specialties, with pathways to expansion of responsibilities, leadership roles, and potential advanced certifications through ongoing education and supported training.
