About You

As an RN Clinical Liaison:

You will report directly to the AVP of Business Development. Your responsibilities include:

  • Ensuring effective and timely intake and referral processes to meet patient, family, and referral source needs
  • Completing admissions for referred patients, including emergency cases and inpatient hospice admissions
  • Providing accurate information about hospice services to inquiries
  • Responding promptly to referral calls and initiating necessary paperwork
  • Advocating for patients by evaluating/developing care plans and obtaining informed consent
  • Engaging with multiple account types such as SNFs, ALFs, hospitals, and physicians
  • Offering in-services and educational sessions on hospice benefits

Hospice RN Clinical Liaison Requirements:

  • Compassionate, kind, and empathetic attitude towards clients, patients, and families
  • RN license in the state(s) of employment
  • Minimum of 2 years of healthcare experience preferred, in settings like hospice, home health, clinical liaison, RN liaison, discharge planning, hospice case management, or RN education
  • Respected and known within your community
