This position is for an Independent Contractor to serve San Lorenzo specifically.

FeldCare Connects is currently seeking a self-motivated Physical Therapist to deliver excellent care and work with adults of all ages.

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs to promote movement, reduce pain, restore function, and prevent disability. Based on this assessment, the therapist develops a treatment plan, executes interventions to improve patient well-being, and monitors progress.

Requirements for a Physical Therapist in Home Health include:

• Strong knowledge of physical therapy principles, theories, and methods applied in rehabilitation.
• Skills in providing various treatments.
• Effective communication skills, both written and oral.
• Ability to understand the role of therapy considering the patient's age, needs, and environment.
• Ability to accurately prepare and maintain records with discretion.
• Sound clinical reasoning in treatment planning, implementation, and monitoring patient progress.
• Ability to recognize when treatment plans need modification and report accordingly.

Qualifications:

• Enthusiasm and ability to work with all ages.
• State licensure and registration as a Physical Therapist.
• Completion of an APTA-approved Physical Therapist curriculum.
