Jupiter Medical Center in Palm Beach County is seeking a licensed social worker to provide psychosocial assessments, counseling and discharge planning for patients and families.

You will collaborate with physicians and the healthcare team, coordinate transitions to the next level of care, and help patients navigate community resources while maintaining compliance with state and federal regulations.

Formación

• Master’s degree in social work required.
• BLS certification through the American Heart Association for Healthcare Providers.
• Highly organized, empathetic, and capable of multitasking in a high-stressed environment.
• Excellent verbal, written, communication, and organizational skills.
• Proficient assessment skills in identifying psychosocial needs and planning transitions.

Responsabilidades

• psychosocial assessments, therapeutic treatments, counseling and discharge planning, care coordination for patients and their family.
• Reviews and discusses the plan with the patient, family, representative and multidisciplinary care team.
• Coordinates planning and evaluation among disciplines responsible for patient care.
• Coordinates family meetings to provide a smooth transition to the next level of care.
