Job summary

Jupiter Medical Center in Palm Beach County is seeking a Nurse Navigator to serve as a single point of contact for referring physicians, patients, and caregivers, providing resources and assistance to overcome treatment barriers and expedite access to clinical and supportive care services.

The Nurse Navigator will prepare and maintain updated treatment plans, assist with appointments and coordination with labs and specialists, and engage in community outreach to promote the program and Jupiter

Qualifications

• Graduate of an accredited school of professional nursing, BSN Preferred
• RN- Current RN licensure in the state of Florida
• American Heart Associations BLS Certification for Healthcare Providers
• Five (5) years of nursing experience with three (3) in Oncology related field preferred
• Must have knowledge of physician office processes.
• Must have excellent verbal and written skills.
• Must be proficient in building relationships and in coordination of all aspects of care.
• Knowledgeable of oncology disease process and symptom management.
• Ability to foster and maintain effective working relationships and communication levels with patients, employees, physicians, administration and the general public.
• Performs independently within his/her scope of practice.
• Ability to interpret, recognize, analyze difficulties in patient care situations.
• Exhibits excellent leadership, organizational, written, and verbal communication and excellent interpersonal skills.

Responsibilities

• Prepare and maintain an updated treatment plan for each patient to synchronize care management between the medical team and patient.
• Assist with patient appointments, including those made with labs, diagnostic facilities, and specialty physicians will be provided.
• Community outreach including possible public speaking and promotion of the program and Jupiter Medical Center system is an essential part of this position.
• Performs other duties as assigned.
