Occupational Therapist, OT - BAYADA Home Health Care

BAYADA Home Health Care is seeking an experienced Occupational Therapist, OT for a full-time or part-time opportunity providing home health visits in our Ohio Visits office . This office services adult and geriatric patients on a per-visit basis throughout the West Side of Cuyahoga County and Medina County .

As a home care Occupational Therapist, you will be a vital part of a multidisciplinary healthcare team that offers rehabilitative care and skilled nursing, enabling clients to receive necessary medical care while remaining at home.

Occupational Therapist, OT Responsibilities :

• Make home visits to clients within designated geographic areas.
• Perform comprehensive evaluations of patients’ functional status, including muscle function, endurance, visual coordination, communication skills, and ADL / IADL performance.
• Assess cognitive, psychological, and motor performance aspects and develop personalized intervention plans for optimal outcomes.
• Evaluate the home environment for hazards or barriers to independent living.
• Identify equipment needs and environmental modifications to improve client outcomes.
• Regularly review and update the occupational therapy care plan and collaborate with nursing in the multidisciplinary care process.
• Educate patients, families, or caregivers on rehabilitative activities to promote health, safety, and independence.
• Document observations, interventions, and evaluations accurately using a state-of-the-art touchpad tablet.

Occupational Therapist, OT Qualifications :

• Current OH Occupational Therapist license.
• Prior home care experience preferred but not required.
• At least one year of recent experience as an Occupational Therapist.
• Graduation from an accredited occupational therapy program (AMA and AOTA recognized).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency in computer skills; experience with electronic medical records (EMR) preferred.
