Position Overview :

The ideal Licensed Practical Nurse (LPN) will have a strong background in patient care, medical documentation, and health coaching. This role involves working closely with patients in long-term care (LTC), memory care, and outpatient facilities.

Key Responsibilities :

• Provide direct patient care under supervision of RNs or physicians.
• Conduct health coaching sessions to promote wellness and chronic condition management.
• Perform accurate medical documentation in compliance with healthcare regulations.
• Assist in outpatient settings, LTC, and hospital environments.
• Administer medications and treatments as prescribed.
• Monitor patients’ vital signs and report changes.
• Support dialysis patients and educate them about the process.
• Use medical terminology to communicate effectively with healthcare teams.
• Conduct diagnostic evaluations to assess health status.
• Ensure infection control protocols and maintain a safe patient environment.

Requirements :

• Active LPN License in VT, NH, or a compact state.
• Minimum 2 years of LTC experience.
• Ability to adapt quickly and work independently.
• Experience in memory care preferred.
• Excellent verbal and written communication skills.
• Ability to collaborate while maintaining confidentiality.
• Flexibility to work in varied healthcare environments and schedules.
