Licensed Practical Nurse, Health Plan Support
The Licensed Practical Nurse, Health Plan Support is an integral part of the Care Management team, responsible for triaging calls requiring urgent follow-up, educating, coordinating, and tracking preventative care with health plan members. This is a fully remote position with periodic travel to the office closest to your location.
Performance Responsibilities and Standards
- Respond to incoming calls to the Care Manager aiming for one-call resolution or same-day resolution.
- Assist and consult with members and providers via phone.
- Gather necessary information for escalation to Care Manager or supervisor.
- Contact health plan members needing preventative care, including immunizations and health screenings, to provide guidance, education, and support alongside the Care Manager.
- Review health reports and member records to identify those needing annual wellness visits or preventative care.
- Schedule and coordinate preventative health care appointments within required timeframes.
- Arrange transportation for wellness or preventative appointments as needed.
- Document all communications in the electronic medical record (EMR).
- Update member records with health maintenance documents from external sources.
- Collaborate with the Care Management Team if preventative care coordination is unsuccessful.
- Demonstrate compassion, collaboration, communication, and good judgment in member care.
- Maintain compliance with quality standards and support members to ensure they feel heard and respected.
- Manage multiple systems simultaneously.
- Perform other duties as assigned to support care management.
This position is remote; candidates must reside within Upstate New York.
Qualifications
- Licensed Practical Nurse with current NY licensure and residence in NYS.
- Minimum of two years of clinical experience and/or experience in outreach, managed care, case management, or ambulatory settings.
- Proficient in Microsoft Office (Excel, Word).
- Familiarity with EMR systems like Mediture/TruChart, HealtheConnections, and communication tools like Zoom and Microsoft Teams.

Physical Requirements
- Ability to sit or stand at a desk most of the day.
- Sufficient visual and hearing ability for communication.
- Effective verbal and written expression.
- Use of telephone and laptop for communication and data entry.
- Bloodborne Pathogens Exposure Category III.
Salary: $21.36 - $26.70/hour, with benefits including 401K, gym, tuition reimbursement, work/life balance, HSA, recognition programs, PTO, holidays, disability, EAP, and more.
