Licensed Practical Nurse, Health Plan Support
The Licensed Practical Nurse, Health Plan Support, is an integral part of the Care Management team, responsible for triaging urgent calls, educating, coordinating, and tracking preventative care with health plan members. This is a fully remote position with periodic travel to the office nearest your location.
Performance Responsibilities and Standards
- Respond to incoming calls with the goal of resolving issues in one call or within the same day.
- Assist and consult with members and providers over the phone.
- Gather information for escalation to Care Manager or supervisor as needed.
- Contact members requiring preventative care, providing clinical guidance, education, and support in conjunction with Care Managers.
- Review health plan reports and records to identify members needing wellness visits or preventative care.
- Coordinate and schedule preventative health care appointments within required time frames.
- Arrange transportation for wellness or preventative appointments as needed.
- Document all communications in the electronic medical record (EMR).
- Update member records with health maintenance documents from external sources.
- Collaborate with the Care Management team if preventative care efforts are unsuccessful.
- Demonstrate compassion, collaboration, communication, and sound member-care judgment.
- Maintain compliance with quality standards and support members to ensure they feel heard and respected.
- Manage multiple systems simultaneously.
- Perform other duties as assigned to support care management.
This position is remote; candidates must reside within Upstate New York.
Qualifications
- Licensed Practical Nurse with current NY licensure and residence in NYS.
- Minimum of two years clinical experience or related field experience such as outreach, managed care, or case management.
- Proficient in Microsoft Office applications.
- Familiarity with EMR systems like Mediture / Truchart, HealtheConnections, and communication tools like Zoom and Microsoft Teams.

- Strong interpersonal skills for effective interaction with patients, team members, and physicians.
- Ability to work independently with attention to detail.
- Maintain high customer service standards.
- Ability to learn and utilize department-specific software.
- Flexible schedule to work as part of a team.
Physical Requirements
- Ability to sit or stand at a desk for 90% of the day.
- Sufficient visual and hearing ability to comprehend communication.
- Verbal and written expression skills.
- Ability to use a telephone effectively.
- Use of a laptop for entering orders and obtaining information.
- Bloodborne Pathogens Exposure Category III – rarely contacts blood or body fluids.
Competitive salary : $21.36 - $26.70 / hour, non-exempt position, with benefits including 401K match, on-site gym, tuition reimbursement, HSA, recognition platform, paid time off, and more.
