Position Overview

The RN Case Manager is responsible for the independent management of the Hospice patient population, requiring advanced assessment, teaching, and decision-making skills. The RN Case Manager cares for a caseload of hospice patients by evaluating their appropriateness for hospice care and developing the plan of care in collaboration with providers, caregivers, and Hospice Interdisciplinary Team members. They provide follow-up by evaluating the effectiveness of the care plan and monitoring patient and caregiver responses. The RN Case Manager administers patient care based on provider orders, departmental policies, regulatory guidelines, and best practices. They collaborate with the provider, patient, caregivers, and Hospice team members to meet care goals, respecting patient limitations and preferences in accordance with hospice philosophy.

Minimum Requirements

1. Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
2. Technical Skills: Nursing skills as defined by generally accepted standards of practice; good interpersonal skills.
3. Licensure/Registration/Certification: Current Texas State license as a Registered Nurse, CPR certified, and current Texas Driver's License with auto insurance. Reliable transportation required.
4. Experience: Two years of experience as a Registered Nurse with med/surg experience.
