The RN Case Manager is responsible for independent management of the Hospice patient population requiring use of advanced assessment, teaching and decision-making skills. The RN Case Manager cares for a caseload of hospice patients by evaluating the patient for appropriateness of hospice care and developing the plan of care in collaboration with the provider, caregivers, and Hospice Interdisciplinary Team members. The RN Case Manager provides follow up by evaluating the effectiveness of the plan of care and monitoring the patient and caregivers response to the plan. The RN Case Manager provides patient care based on provider's orders, departmental policies and procedures, regulatory guidelines and best practice. The RN Case Manager provides care in collaboration with the provider, patient caregivers, and Hospice Interdisciplinary team members to meet the goals of care with the consideration of the patient's limitations and personal preferences in accordance with hospice philosophy.
Other Items of Note:
Required Education:
Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
Required Technical Skills:
Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
Required Licensure /Registration /Certification:
Current Texas State license as a Registered Nurse, CPR certified, and current Texas Drivers License and auto insurance.Must have reliable transportation.
Required Experience:
Two years experience as a Registered Nurse with med/surg experience.
