Job summary

Total Life is seeking nurse practitioners for an in-person contract role in the Lodi-Sacramento area to lead longevity days at partner senior living communities. You’ll open with an educational talk and then conduct 45-60 minute longevity visits built on a comprehensive geriatric assessment.

You’ll be trained by our Chief Medical Officer and will coordinate with primary care providers, with Medicare documentation to be completed in our EHR.

Qualifications

• Active, unrestricted California RN license.
• NP Furnishing Number from California BRN.
• Current national NP board certification.
• Active DEA registration for California.

Responsibilities

• Conduct 45–60 minute longevity visits built on a comprehensive geriatric assessment including medication review, cognition, mood, sleep, nutrition and function.
• Evaluate eligibility for weight management and GLP-1 therapy where appropriate.
• Coordinate follow-up care with residents' primary care providers and specialists as needed.
• Document every encounter to Medicare standards in the EHR.
• Represent Total Life professionally with residents, families and community staff.
