Y.A.P.A. Apartment Living Program Inc is seeking a full-time Case Manager in Umbria, Philadelphia, PA.

The role involves developing and implementing treatment plan goals, working with a multidisciplinary team, and providing direct oversight for Members as they transition to less structured living arrangements. Key responsibilities include admissions support, incident reporting, weekly case management, and on-call crisis management.

Competenze

• High School Diploma with 3 years' experience in Mental Health/Behavioral Health/Drug and Alcohol field or an Associates degree with 2+ years' experience.
• Must possess a valid, non-commercial driver's license with a clean driving record
• Excellent communication, interpersonal, organizational, and time management skills.
• Demonstrated ability to work as part of a multi-disciplinary team.

Mansioni

• Collaborate with admissions to support admission and acclimation of new Members.
• Complete Incident Reports per MCO, State, and policy requirements.
• Develop treatment plans with the multidisciplinary team.
• Complete weekly case management sessions and document in DAP format.
• Facilitate groups and provide documentation.
• Transport Members to appointments as directed.
• Observe and document drug/alcohol screenings as applicable.
• Provide crisis coaching and on-call support as needed.
• Ensure all Member documentation is completed within 24 hours.
