We are looking for an aged care health professional with community care experience to be our Care Manager. The Care Manager will join our team along with other Care Managers and the current nursing team. Our services include aged and disability care, transitional care, hospital in the home, and palliative care. We are seeking experienced Nurses who are enthusiastic and passionate about providing caring, compassionate, respectful, and high-quality service. Our goal is to help our clients live empowered and independently at home.

Qualifications
  • Health care professional with experience in Community and Aged Care
  • Sound understanding of My Aged Care, Aged Care Reforms, and Consumer Directed Care principles
  • Comprehensive assessment and case management experience
  • Experience providing care and services to people from culturally and linguistically diverse backgrounds

Commitment to work within RAH Mission, Vision, and Values.

  • Capacity to take initiative and be innovative
  • Highly developed organizational, administrative, and financial management skills with the ability to determine priorities, coordinate activities, and meet deadlines
  • Excellent communication and interpersonal skills, both written and oral, and strong computer literacy with Microsoft and database applications
