Job summary

Cigma Medical Coding in Coimbatore is seeking a dedicated HCC coder with 1–5 years of experience to assign ICD-10-CM and CPT codes for risk adjustment. You will ensure documentation supports codes and adhere to CMS guidelines.

Certification CPC or equivalent is preferred. This is a full-time, on-site role with direct walk-in interviews. Up to 20 vacancies available, with compensation based on last CTC.

Qualifications

• 1–5 years’ experience in HCC coding.
• Certification: CPC or Equivalent.
• Excellent knowledge in Anatomy.
• Good knowledge of medical coding systems, terminologies, regulatory requirements, auditing concepts and principles.

Responsibilities

• Assign codes to diagnoses and procedures using ICD and CPT codes.
• Accurately assign ICD-10-CM codes that map to HCCs for risk adjustment.
• Apply CMS guidelines to ensure codes reflect patient conditions.
• Ensure codes are supported by clinical documentation.
• Maintain high accuracy and productivity in coding and auditing.
• Perform audits of coded records for quality and compliance.
• Verify that documentation supports the assigned codes.
• Identify coding errors during audits.

Job Descriptions:

• Assign codes to diagnoses and procedures, using ICD (International Classification of Diseases) and CPT (Current Procedural Terminology) codes
• Accurately assign ICD-10-CM codes that map to Hierarchical Condition Categories (HCCs) for risk adjustment.
• Apply official coding and risk adjustment guidelines, including those from CMS, to ensure codes correctly reflect patient conditions.
• Ensure that all assigned codes are supported by the clinical documentation in the record.
• Maintain high accuracy and productivity standards for coding and auditing.
• Perform retrospective, concurrent, and prospective audits of coded records to ensure quality and compliance.
• Verify that physician documentation supports the assigned diagnosis codes.
• Identify coding errors, discrepancies, and inconsistencies during audits.
