Job summary

Total Care Therapy is seeking a skilled Speech-Language Pathologist to evaluate, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders in older adults across assisted living and independent living communities in Central Indiana.

You will develop individualized treatment plans, perform dysphagia evaluations, document in the EMR, and collaborate with an interdisciplinary team to deliver patient-centered care.

Qualifications

• Current State Speech-Language Pathologist (SLP) License.
• CCC-SLP preferred (or eligibility).
• Current CPR/BLS Certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized speech therapy plans of care.
• Assess and treat speech, language, voice, cognitive-communication, and swallowing disorders.
• Perform dysphagia evaluations and implement appropriate swallowing interventions.
• Reassess patient progress and adjust treatment plans as needed.
• Complete timely and accurate documentation using the EMR.
• Educate residents, caregivers, and facility staff on communication strategies and home exercise programs.
• Collaborate with physicians, nursing staff, dietitians, and interdisciplinary team members to ensure coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering compassionate, evidence-based therapy services.

Compensation:Per-Visit to Salary(based on years of experience)

Scheduling:Monday-Friday Full-Time (Weekends available but not required)

About the Role

As a Speech-Language Pathologist, you will evaluate, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders while developing individualized treatment plans that improve residents' quality of life and functional independence.

Responsibilities

• Evaluate residents and develop individualized speech therapy plans of care.
• Assess and treat speech, language, voice, cognitive-communication, and swallowing disorders.
• Perform dysphagia evaluations and implement appropriate swallowing interventions.
• Reassess patient progress and adjust treatment plans as needed.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on communication strategies, swallowing precautions, and home exercise programs.
• Collaborate with physicians, nursing staff, dietitians, and interdisciplinary team members to ensure coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering compassionate, evidence-based therapy services.

• Current Certificate of Clinical Competence (CCC-SLP) preferred (or eligibility if applicable)
• Current CPR/BLS Certification
