Job summary

Qualifications

  • Diploma or Bachelor in Pharmacy with active Karnataka Pharmacy Council license and 1–2 years of experience in retail pharmacy or hospital formulary.
  • Knowledge of generic names and substitutes; familiarity with Indian Pharmacopeia (IP).
  • Ability to compute dosages and manage pharmacy software & inventory.

Responsibilities

  • Dispense prescribed pharmaceuticals and ensure drug receipt, preparation and accountability per regulations and hospital policy.
  • Maintain narcotics register, conduct monthly stock checks, and ensure FIFO in dispensing.
  • Process ADR forms with doctors and update HMIS with rate changes and GRN bills.
  • Follow-up with patients for subsequent purchases and maintain proper documentation.
  • Supervise dispensing according to protocol and manage patient relationships.
  • Maintain all pharmacy registers, inventory records, and compliance with state pharmacy rules.

Job description

Designation: Pharmacist Qualifications & Experience:

Diploma or Bachelor in Pharmacy with active Karnataka Pharmacy Council license and at least 1-2 years of experience in retail pharmacy or hospital formulary.

Reporting to: Pharmacy In charge Knowledge

Should have knowledge of all generic names of drugs with their substitute brands available in market and a familiarity with Indian Pharmacopeia [IP].

Knowledge of computers, pharmacy software and mathematics to compute dosages and rate of drug use.

Demonstrating knowledge of pharmaceutical ordering procedure, pharmaceutical storage and maintenance

Skills;

Good communication skills & interpersonal relationship.

Should have problem solving abilities to help and guide the patients.

Key Result Areas

To dispense all necessary pharmaceuticals and consumables prescribed by the physicians and ensures that all aspects of drug receipt, preparation and accountability are conducted in accordance with all applicable regulations, protocols and hospital policies.

To achieve dispensing accuracy so that errors will be less than 1/500 [i.e. 0.0002%]

Specific Responsibilities

Updating narcotic register on daily basis.

Stock check in wards every month.

Issuing ADR forms to the doctors for completing the ADR reporting.

Posting GRN bills in HMIS and send mails to HMIS if any rate changes found.

Follow-up should be given to all patients for next purchase of medication as per prescription.

Ensure FIFO method is followed in all dispensing and arrangements of medications and consumables inside the pharmacy.

The storing and maintenance of pharmaceuticals and the compliance with inventory management rules & pharmacy protocol.

Supervise patient relationship and service.

Supervise & ensure dispensing of drugs according to protocol.

General Responsibilities
  • Maintain knowledge of all generic names of drugs with their substitute’s brands available in the pharmacy.
  • To adhere protocols of pharmacy :
  • Following physicians’ orders, prescriptions from patients, compounds and dispenses medicines and other pharmaceuticals.
  • Ultimately responsibility for all aspects of the completed prescription and assumption of the responsibility for the filled prescription such as accuracy of drug, strength, batch, expiry, labeling, proper container and other requirements.
  • Utilizes knowledge of age specific needs of patient.
  • Maintains strong knowledge of hospital policies, government regulations and state board of pharmacy regulations and dispenses accordingly. Comply with all state board of pharmacy rules and licensing regulations.
  • Management of bills payable & receivables. Monitors, records and communicates as appropriate utilizing computerized documentation systems.
  • Duty to maintain adequate knowledge about dosage and ability of new medications, communicates with patients in terms that are understandable.
  • Responsibilities include constantly studying and about the properties and side effects of new drugs and communicating this information to doctors.
  • Adhere to all HR practices & hospital rules; adjust work assignments and schedules allotted by the Head of department.
  • Responsible & capable to maintain pharmacy in absence of superiors.
  • Maintain documentation of the CQIs and updating the same.
  • Responsible for taking feedback and Act on the feedback received on the patient feedback system “Treat well”.
  • Should be aware about all narcotics and schedule drugs policies
  • Prepare the transplant sales report, A and B class reports.
  • Removing the non moving and near exp medications as per audit.
  • Maintain the proper stock in pharmacy.
  • Handover of any issues and work to the next shift staff with proper documentation.
  • Submitting the receipts of local purchase to accounts with proper voucher.
  • Issue the medication or consumables against the indents.
  • Collecting data for schedule drugs and TB drugs.
  • Clearing pending indents and OT bill pending for IP patients.
  • Maintain all pharmacy related register and records.
  • Checking the consumption of stationary.
  • Involvement in incident report system.
Participate

Participate & consult with physicians and nursing staff regarding patient reactions, errors, queries and complaints.

Participate & help in pharmacy audit & ordering of pharmaceuticals.

Participate & receive extensive training by the Head of Department to become a capable pharmacist.

Participate in internal department staff review meetings.

Ensure teamwork & a healthy work environment among the employees of the department

Participate in maintaining the cleanliness and safety of all hospital areas.
