Crossing Hurdles is seeking an experienced physician to review medical records for AI training and data quality improvement. The role focuses on inpatient adult medicine notes, including HPI, PMH, medications, social history, assessments, and plans.

You will verify information, identify inconsistencies, and provide actionable feedback to enhance model performance. Ideal candidates hold an MD/DO with Internal Medicine board certification and have strong experience in chart review, documentation

Qualificações

• MD or DO with Internal Medicine board certification.
• Strong understanding of inpatient documentation and hospital workflows.
• Comfort reviewing clinical notes and raw medical record data.
• Experience with chart review, clinical documentation improvement, utilization review, or medical coding/QA is a plus.

Responsabilidades

• Review medical records, including FHIR data and clinical notes, for AI training.
• Check whether clinical information and claims are supported by the medical record.
• Review inpatient adult medicine documentation (HPI, PMH, meds reconciliation, social history, assessments, plans).
• Identify missing, incorrect, or inconsistent information in medical records (dosing, interactions, age-specific details).
• Share feedback to help improve AI models and data quality.

Requirements

• Have strong relevant experience in the domain.
• MD or DO, board certified in Internal Medicine (or Internal Medicine with a Hospital Medicine focused practice designation).
• Strong understanding of inpatient documentation and hospital workflows, including admission, daily progress notes, transitions of care, and discharge.
• Comfortable reviewing clinical notes and raw medical record data.
• Experience with chart review, clinical documentation improvement, utilization review, or medical coding/QA is a plus.
