Role Description

The Home Health Nurse will provide direct nursing care to patients in their homes, ensuring safe, compassionate, and high-quality clinical support. Daily responsibilities include assessing patient conditions, administering medications, monitoring vital signs, managing wound care, and coordinating with physicians and other healthcare professionals as needed. The nurse will educate patients and families on treatment plans, home care procedures, and preventive health practices, while maintaining accurate medical records and documentation. This is a full-time, on-site role based in Tamil Nadu, India, requiring regular travel to patient residences within the assigned area.

Qualifications

• Strong clinical foundation with skills in Nursing and Medicine appropriate for home health settings.
• Experience in Home Care and Patient Care, including monitoring conditions and assisting with daily living activities.
• Competence in Wound Care, including dressing changes, infection prevention, and follow-up monitoring.
• Recognized nursing qualification (GNM, B.Sc Nursing or equivalent) and valid registration with the relevant nursing council.
• Ability to work independently in the field, manage schedules, and communicate clearly with patients, families, and healthcare teams.
• Knowledge of basic medical equipment used in home settings and adherence to safety and infection control protocols.
• Comfort working in diverse home environments, with a respectful, culturally sensitive, and patient-centered approach.
• Prior experience in home health or community nursing in India is an advantage.
