Job summary

Heaps Health Solutions India is seeking a teleconsultation healthcare coach to engage with patients by calls and messages to collect medical histories, monitor status, and guide adherence to prescribed plans.

You will provide psychosocial support, monitor progress, and answer queries from FAQs while coaching on labs, medications, and diet. Collaboration with the Program Manager helps identify trends for better patient care.

Responsibilities

• Engage with patients over calls to understand health status and history.
• Identify further requirements such as meds, labs, or specialist referrals.
• Ensure patients provide complete medical history and align with treatment plans.
• Provide psychosocial support to cope with illnesses.
• Monitor progress and steer conversations if alarming symptoms appear.
• Handle patient queries using pre-defined FAQs.
• Coach patients to remain compliant with labs, meds, and diet.
• Develop and implement personalized care plans for high-risk patients.
• Provide ongoing support via phone, video, and messaging.
• Educate on risk reduction, healthy living, and self-care.
• Offer wellness and diet tips as needed.
• Identify symptoms or deviations from care path and address them.
• Collaborate with Program Manager to identify trends and improve compliance.

Job description

Role & responsibilities

• Engage with the patients over various calls to understand their current medical health status, past medical/surgical history, medications taken, labs done etc.
• Understand if the patient has any additional requirements medications, labs, dietitian, physiotherapists etc.
• To effectively engage the patients in giving complete medical history and determine if they have any deviations from the recommended treatment plan
• Provide patients with the psychosocial support needed to cope with chronic, acute or terminal illnesses
• Monitor patients to evaluate the progress of their health conditions and probe them in the correct direction in case of any alarming symptoms reported
• To smartly handle patient queries and provide responses from pre-defined FAQs.
• Understand if the patient is compliant with Labs, Medications, and Diet & coach them effectively in order to be compliant with the prescribed treatment plan as provided by their treating physician
• Develop and implement individualized care plans that address the unique needs of high- risk patients.
• Provide continuous support, motivation, and education to patients through regular check- ins via phone, video calls, and messaging.
• Educate patients on risk reduction strategies, healthy lifestyle changes, medication adherence, and self-care techniques
• Provide wellness, lifestyle and diet tips wherever deemed necessary
• Proactively identify if the patient is showing any symptoms or deviation from his care path
• To collaboratively work with the Program Manager in identifying new trends, and new insights and effectively managing patient compliance
