Praxis Care is seeking a Support Worker to empower individuals in independent living and to follow person-centred plans. You will work with Team Leaders to implement tailored support programmes, improving daily living skills and safety within the service.

All advertised salaries are based on 39 hours (FTE) and will be pro rata to contracted hours.**
