Responsibilities

• Assess, plan, implement and evaluate individualized, person-centred care plans within an agreed framework
• Provide holistic, person-centred care to promote independence and quality of life
• Develop and maintain strong relationships with residents and their families/guardians
• Collaborate with multidisciplinary team for optimal care and outcomes
• Maintain accurate, timely documentation and reports relating to resident care
• Promote quality care using evidence-based practices within a safe environment
• Support resident autonomy, rights and informed consent
• Act as advocate for residents and families, guiding decisions and community participation
