Job summary

Delta Centre CLG in Carlow, Ireland, is seeking a Social Care Practitioner to provide day-to-day personal care and support for residents at Delta Centre, promoting dignity, independence and person-centred planning.

This full-time role requires Level 7 or above qualification in a health-related discipline and experience in social care, with strong communication and record-keeping. A driving licence is desirable for broader duties.

Qualifications

• Experience in social care is required.
• Proven ability to interpret and apply relevant standards and policies.
• Excellent communication and motivation to support adults with intellectual disabilities.
• Strong record-keeping and documentation skills.
• Willingness to work within HIQA/HSE safeguarding guidelines.

Responsibilities

• Provide day-to-day personal care and support for Residents.
• Ensure Residents experience dignity, respect and choices.
• Review and implement Person-Centred Plans for each resident.
• Participate in regular reviews of care protocols and plans.
• Facilitate residents' communication needs and independence.
• Help residents identify future goals and document support needs.
