Qualifications

• QQI Level 5 in Healthcare Support or equivalent, with minimum 1 year relevant experience.
• Candidates who have completed at least 1 year of Nursing or Social Care degree will be considered.
• Strong passion for promoting and empowering those you support.
• Excellent knowledge of HIQA regs, Disability Act 2005, Health & Safety, Data Protection & GDPR.
• Excellent verbal and written communication to engage with individuals, families, colleagues and managers.
• Ability to document and report accurately and professionally.
• Ability to plan, coordinate and document daily activities and individualised care plans.
• Ability to maintain accurate digital records and GDPR-compliant reports.
• Strong time management to balance multiple responsibilities.
• Rights-based, person-centred approaches to support and advocacy.

Responsibilities

• Empower individuals by planning goals addressing their personal, social, recreational, academic and independent living needs.
• Guide individuals to realise aspirations and promote independence and inclusion.
• Collaborate with internal and external services; build relationships with families for holistic support.
• Maintain accurate records and complete reports to required standards; provide healthcare support.
• Facilitate life skills development through hobbies and activities, e.g., budgeting, cooking, time management.
• Work with healthcare professionals to create care plans responsive to evolving needs.
