Job summary

Rehab Group in Kilkenny invites applications for a Relief Programme Facilitator. You will work within a professional team delivering learning, social and recreational activities to people with disabilities, supporting valued community participation and independence.

The role emphasises person-centred planning and adaptive supports. Minimum QQI Level 5 in Social Care is required; a third level Health & Social Care qualification is desirable.

Qualifications

  • Minimum QQI Level 5 in Social Care or equivalent.
  • A third level graduate in Health & Social Care or equivalent is desirable.
  • Experience supporting adults with intellectual disabilities.

Responsibilities

  • Be a key worker to service users to facilitate Person Centred Planning.
  • Identify goals and supports for each service user and maintain records.
  • Work with service users on day-to-day support needs.
  • Develop and deliver person-centred programmes and maintain records.
  • Support individuals and groups to access their community through activities.

About the Role:

To be part of a professional team & progressive service supporting individuals with complex challenges to achieve and maintain valued roles in their community through learning, social and recreational opportunities. The successful candidate will be required to work in a way which reflects the values of Supported Self Directed Living, implementing the core values of New Directions. The candidate will have an innovative ability, together with drive, enthusiasm, motivation and a positive attitude. To facilitate individuals with their programme choices and demonstrate a passion to assist in enhancing the person's image in all settings. To facilitate programmes and projects that include educational, developmental, recreational and occupational themes and activities of the service, as appropriate. The candidate will work as a positive member of the team and make a conscious effort to investigate new opportunities and advocate the same for the best interest of the service users, service and team

What we're looking for:

Minimum QQI Level 5 in Social Care or equivalent A third level graduate, Health & Social Care or equivalent is desirable. Experience supporting adults with intellectual disabilities as a primary diagnosis. Programme facilitation skills an advantage

Your Responsibilities
  • To be a key worker to service users in order to facilitate the Person Centred Planning process.
  • To identify goals and supports required by individual service users and maintain records as required.
  • To work with service users with their day to day support needs
  • To develop and deliver person centred programmes to meet the needs of service users as identified through Person Centred Planning and to maintain the required records.
  • To support individuals and groups in accessing their community to meet identified needs by facilitating activities and programmes and any other reasonable duties as deemed appropriate by the Programmes Supervisor / Community Services Manager.

We offer a competitive starting salary of € 16.38 per hour.
