The successful candidate will manage a varied adult caseload within a community setting, providing high-quality, evidence-based physiotherapy assessment and intervention aimed at promoting independence, function, and quality of life.

The role includes working with adults presenting with a broad range of musculoskeletal (MSK) conditions, particularly within the older population, including age-related degenerative conditions, falls risk, reduced mobility, osteoarthritis, osteoporosis, balance impairment, and post-orthopaedic rehabilitation.

This is an excellent opportunity for a clinician who enjoys autonomous practice while working collaboratively within a multidisciplinary Primary Care team.

Key Responsibilities of the Physiotherapist:
  • Deliver comprehensive physiotherapy assessment and treatment to adults within a Primary Care setting.
  • Manage a diverse caseload including musculoskeletal conditions, chronic disease management, rehabilitation, and age-related conditions.
  • Assess and treat older adults with common MSK presentations, including osteoarthritis, spinal pain, joint disorders, mobility decline, osteoporosis, frailty, and post-fracture or post-operative rehabilitation.
  • Develop individualised rehabilitation programmes aimed at improving strength, balance, mobility, pain management, and functional independence in the elderly population.
  • Provide falls prevention assessment and intervention, promoting safe mobility and reducing the risk of future injury.
  • Provide home visits as required to support patients with mobility or functional limitations.
  • Contribute to discharge planning and early intervention strategies to prevent hospital admission and support ageing in place.
  • Work collaboratively with GPs, occupational therapists, public health nurses, social workers, and other members of the MDT.
  • Educate patients, families, and carers on self-management strategies, exercise programmes, and long-term management of MSK conditions.
  • Maintain accurate clinical documentation in line with professional and organisational standards.
  • Participate in service development, audit, and quality improvement initiatives.
Essential Criteria for the Physiotherapist:
  • BSc (or equivalent) in Physiotherapy.
  • Current CORU registration.
  • Experience working with adult caseloads in community or Primary Care settings.
  • Experience assessing and managing musculoskeletal conditions, with particular interest or experience in elderly rehabilitation, falls prevention, and age-related MSK disorders.
  • Strong clinical reasoning and independent decision-making skills.
  • Excellent communication and organisational abilities.
  • Full clean driving licence and access to own transport (as the role involves community travel).
