Job summary

The Rehab Group is seeking a Relief Programmes Facilitator in Longford. The role involves facilitating service users in person‑centred programmes including independent living, development and leisure activities within a quality‑of‑life framework.

Reporting to the Community Service Manager, you will work as a key worker, develop tailored programmes, and support community access while maintaining records and adhering to safety guidelines.

Qualifications

• Minimum 1 year experience in a relevant setting, facilitating person-centred plans.
• Experience in designing, developing and providing programmes and activities to meet service users' needs.
• Experience in lone working and community support.
• Experience with autism, mental health and physical disabilities.
• Interest in services for mental health needs.

Responsibilities

• Be a key worker to service users to facilitate Person Centred Planning and maintain records.
• Develop and deliver person-centred programmes and keep required records.
• Support service users to access community activities and meet identified needs.
• Work as a lone worker and liaise with team and external agencies.
• Promote independence and community integration in line with service user choices.
• Follow Rehab Group safety rules and procedures at all times.
• Promote safety, hygiene and good housekeeping within the service.
• Report concerns to supervisor as appropriate.
