Job summary

Focus Ireland, based in Dublin, is seeking a Case Manager for Aylward Green to deliver intensive one-to-one family support under a Specific Purpose contract. The role works within an Intensive Care and Case Management Model to assist families facing chronic exclusion.

You will collaborate with multi-disciplinary teams, coordinate with Tusla and other agencies, and deliver flexible, family-centered interventions to empower parents and children toward positive outcomes.

Qualifications

• A third-level qualification at degree level in Social Care or related area and/or CORU-registered Social Care Worker.
• 2 years of experience with high need families using intensive case management and family support.
• Experience with customers who have co-existing conditions.
• Experience of liaising with other agencies.
• Strong report writing abilities.
• Understanding of issues for high-need families with homelessness.
• Experience working in social care or clinical teams.

Responsibilities

• Work within Focus Ireland values, policies and service standards to deliver high-quality family support.
• Collaborate as part of a multi-disciplinary and multi-agency team for integrated service delivery.
• Provide flexible, family-centred support to parents and children.
• Deliver intensive one-to-one family support interventions toward agreed outcomes.
• Contribute to design and review of individualised support packages with colleagues.
• Develop partnerships with Tusla, HSE, Probation, schools and community organisations.
• Engage stakeholders in the lives of children, parents and customers.
• Participate in case conferences and multi-agency meetings and follow actions.
• Support parents via education, coaching and mentoring for life skills.
• Maintain needs assessments and case records using Homeless Executive tools and recording systems.
• Identify issues contributing to homelessness and support sustainable change.
• Adhere to Focus Ireland policies and participate in meetings and supervision.
