Job summary

Acquired Brain Injury Ireland in Navan, Co Meath, is seeking a Community & Case Manager to support adults with acquired brain injury. This permanent, full-time role (39 hours) focuses on person-centred care, independence, and community participation.

You will lead case management, coordinate with interdisciplinary teams, manage budgets, and contribute to service development while fostering strong relationships with clients and families in the catchment area.

Qualifications

• Relevant third-level qualification in Human Services
• Experience in care or support settings with individuals and families
• Strong written and verbal communications, report writing and presentations
• Ability to lead and develop staff teams
• Ability to build positive relationships with stakeholders
• High attention to detail, practical hands-on approach
• Excellent time management and customer service focus
• Experience in budgeting, forecasting and planning

Responsibilities

• Lead case management and care planning for adults with ABI
• Support clients in achieving independence and community integration
• Coordinate with teams and partner agencies to deliver joined-up services
• Manage staff rotas, supervision, and service operations
• Oversee budgets, payroll, and purchasing for the local service
• Ensure compliance with organisational policies and quality standards
• Promote community links and access to employment, leisure, and supports
• Represent the organisation and contribute to service development
