Job summary

Focus Ireland in Limerick is recruiting a Case Manager for the Housing First service to deliver a person‑centred approach to individuals experiencing long‑term homelessness in Limerick City. The role is a full‑time, specific purpose contract within a multidisciplinary team.

You will complete assessments, develop support plans and maintain records to help clients secure and sustain housing, access health and social services, and pursue education or training opportunities.

Qualifications

• A degree or equivalent in social care (youth work, social studies, addiction, psychology, etc.).
• CORU registration as a Social Care Worker is required or advantageous.
• Experience with homelessness clients and dual diagnosis is desirable.
• Ability to work in a multidisciplinary team and document proceedings clearly.
• Full driver’s license is desirable.

Responsibilities

• Deliver Housing First case management in line with principles and fidelity.
• Work within service policies and best practice standards.
• Build trusting relationships with clients experiencing homelessness.
• Provide a safe, welcoming environment and pursue housing stability for clients.
• Complete assessments and support plans using standard tools and protocols.
• Assist clients to access housing and related services.
• Support tenancy sustainment and engagement with housing providers.
• Provide information on welfare rights and entitlements.
• Support clients in accessing healthcare, counselling, and other services.
• Promote client integration into communities and personal development opportunities.
• Maintain accurate caseload records and documentation in PASS and internal systems.
• Collaborate with multidisciplinary teams and partner agencies to develop and evaluate services.
